2010 Renewal Information

Yearly dues and renewal are required form all levels of membership.

For the year 2010

Annual dues are $50.00 for General and Accredited Members.  Invoices will be mailed Feb-March 2010.  

Annual dues for Individual Members are $25.00.  Renewal date is one year from issue date.

 

Accredited Members are required to submit the following:

     Annual Dues - Check made payable to UMAP

     UMAP Accreditation Annual Update 2010  (word)  (pdf)

     Staff Credential Report   (word)  (pdf)

     Explanation as to how program staff will meet the new in-service requirement

 

General Members are required to submit the following:
